Transportation Appeal
This form is required to appeal for transportation from either remote learning to in-person or parent transportation to District transportation. Please read the form carefully. Transportation may not be guaranteed. If your student is changing learning platforms (in-person/remote), please be sure to file the appropriate appeal with the school principal.

Transportation Change Request (Daycare Form)
Request form to change a student's transportation anytime during the school year. Please submit at least three to five (3-5) days in advance for processing. Submit annually by July 31st to start transportation for the beginning of the new school year. No further chagnes for the new school year can be made two weeks prior to school starting.
We only transport to daycare centers and private sitters within the district.

Change of Address
Required for students that have moved within the district. Two (2) proofs of residency are required when submitting this form.

Elementary Late Bus Authorization
Transportation request for after-school bus transportation for elementary students between third and fifth grades only. Late run nights are Tuesday through Thursdays. We can only transport to one designated location on those late run nights. Estimated drop times may vary depending on the number of students on each bus.

Request for Transportation to Non-Public Schools
Request form for transportation to a non-public school. Requests for the following school year are due annually by the first Monday in April. Two proofs of residency are required. Birth certificates are required for new students transported by the district. Please note that only non-public schools that are 15 miles from a student's primary residence (most direct route) are eligible for transportation services.
